Is my hawk-i dental plan affected?

No, the dental benefits will remain the same and Delta Dental of Iowa will be the carrier.

Why do I have to choose a new Health Plan for my child?

The State of Iowa has contracted new care options for the hawk-i program April 1, 2016.

When do I have to choose a new health plan for my child?

You may change your child's MCO for any reason until June 16, 2016. You mail e-mail your selection to hawkiplanchoice@dhs.state.ia.us or leave us a message at 1-800 257-8563.

My child’s doctor does not accept any of the new health plan options. What should I do now?

You may encourage your doctor to enroll in the network of the new health plans options. You may need to choose another doctor in order to receive services through the hawk-i program if your current doctor chooses not to enroll in a network.

My child receives case management or special authorization on my current hawk-i plan. Which plan offers the same services?

Please refer to the comparison chart enclosed in your enrollment packet in order to make the best possible choice for your child’s care. Also, please seek advice from your doctor before selecting an option.

Is hawk-i going away?

No, the hawk-i Program will remain the same.

Is hawk-i the same as Medicaid now?

No, the eligibility requirements for the program are not changing; the change only involves the health plan options hawk-i offers to your child.

Will my new enrollment in the new health plan option last for 12 months?

No, it will cover your current approved hawk-i eligibility period and then you will be sent a review form to establish a new eligibility year. If approved at renewal time your coverage will continue forward unless you opt to choose a new plan at that time.
